Hospice Care Options
Our 4 Levels of Care:
Routine Home Care
Most hospice care happens here. Our team visits your loved one where they live — at home, in assisted living, or in a nursing facility — to provide comfort, support, and symptom management.
Continuous Care
If symptoms become difficult to manage, a nurse can stay at the home for extended hours to help stabilize your loved one and keep them comfortable.
Inpatient Care
When pain or symptoms cannot be controlled at home, care can be provided in a hospital or inpatient hospice unit until your loved one is stable and comfortable again.
Respite Care
Short-term care (up to five days) in a facility to give family caregivers a much-needed break while your loved one continues to receive professional care.

Common Conditions For Hospice Care
Hospice care is appropriate for individuals facing many types of life-limiting illnesses. Common conditions include:
Cancer (any stage IV or metastatic diagnosis)
Heart disease (including congestive heart failure)
Chronic Obstructive Pulmonary Disease (COPD) and advanced lung disease
